Pain when you ejaculate alongside blood in your semen is a reason to see a clinician. Mayo Clinic lists pain with urination or ejaculation among the reasons to call [Mayo Clinic].
Likely causes
- Prostatitis, inflammation of the prostate, often from infection. Pain when ejaculating is a listed symptom [NHS]. See prostatitis.
- Epididymitis or orchitis, inflammation of the tube behind the testicle or the testicle itself, often from an STI in younger men or a urine infection in older men [NHS].
- STIs such as chlamydia and gonorrhea [NHS].
- Seminal vesicle stones or cysts, less commonly [review].
When it's urgent
Fever or shivering with pain, inability to pass urine, or sudden severe testicle pain need same-day care [NHS].
What helps
Treatment depends on the cause: antibiotics for bacterial infection, and pain relief such as paracetamol or ibuprofen as advised by the NHS for prostatitis [NHS].
